HALL OF FAME NOMINATION FORM – AMATEUR, BUILDER, TEAM CATEGORIES
The Ukrainian Sports Museum and Hall of Fame is looking to celebrate the best candidates possible to celebrate. We are looking for candidates who have a history of participation with a sports organization, who have made incredible contributions to their communities through sport. Candidates can be former athletes, coaches, sports organization leaders and founders, teams who had had a large Ukrainian component, etc.
Each Nominee should be submitted on a separate Word document “sheet” (so that it will be easier to compile and distribute nominations to electors electronically). Please fill in this page for each nominee and send it to UkrSportHOF@gmail.com by April 1, 2018.
If a team is nominated, please include the team name, the sports club, the sport and the specific year’s team under consideration. For example, “Philadelphia Ukrainian Nationals – Soccer – 1962”. A roster of the team would be needed as soon as possible, but need not initially delay the nomination process.
Please send along a high-resolution photograph that could be used in our annual Hall of Fame Induction Book, to be included along with a bio. (Inductees are limited to one page, so longer submissions may be edited as needed.)
The Hall of Fame Induction Ceremony will be held in September.
